We must first determine if the client requires assistance with daily tasks, clinical care, or both. This is determined by an initial assessment that we are available to complete within 48 hours of receiving your inquiry. Following the initial assessment, a care meeting will be conducted to determine the nature and level of care your loved one may need.
Following the initial assessment, a care plan meeting will be conducted to determine the nature and level of care needed. This care plan meeting is an interactive process between the assessor and you. The final care plan is a product of that collaboration and agreed upon by you.
Our clinical director will make routine visits to each client’s home to ensure the plan of care is being met. Documentation is required to be completed daily for each client and random audits are conducted monthly. Our methods of supervision also key us to potential care plan adjustments that may be needed to ensure the finest quality of care.
Following the initial assessment, we identify caregivers matching each client’s needs. We then pair connect each client with each caregiver to ensure the best possible match. The development of trust is essential in establishing the finest quality of care.
